What does a Hammer need to thrive???

Food wise?
Water Flow?
If you've read posts of mine, you might recognize that my water isn't all that great....
Nitrite - 0
Nitrate - 10- 15
Phosphate - .5
Calcium - was 450, last test 400
We're trying really hard to get the levels right... but we're not doing so great. <sigh>
Everything else looks amazing... the fish are ok, the bubbles are huge and growing, the mushrooms are in desperate need of fragging because they're so huge.. and that damn crab is getting bigger everday....
My poor Hammer, (2 heads) is not happy.
I know my water levels are affecting it... but I'm more interested in what it needs to be happy in good water levels...

Feed it phytofeast and DT's, thats what I feed all of my corals including all of my LPS. It needs sufficient light (PC, T-5, or metal halide)

Sadly, one of the heads of the hammer is no longer there.
I cannot figure out what happened. It's gone. I haven't had anything die before and assumed a coral wouldn't just disapear. Could something have eaten it?
It wasn't doing good so I moved it to a different part of my tank and this morning the 2nd one is doing a tiny bit better, but the 1st one is totally gone.
Any ideas?
I think I need to replace my lighting.. I'm not sure when they were replaced before.
Also, I'm realizing I haven't been feeding my tank well enough. I've only been using Formula 1 and Formula 2 (mixed) and frozen Brine Shrimp.
Poor Hammer.
