what happened to my clown

Does anyone have any ideas on what may have happened to my perc. clown (tank raised) I have had him since 10/26/05. He was acting fine until yesterday. He didn't eat in the morning, he was just hanging out on the bottom by my mushrooms. When I came home from work he was basically doing the same thing. Ate a little bit when I fed them in the evening. Again this A.M. the same thing swimming close to bottom near the mushrooms. Was gone most of the day. When I checked on him about 8:00 he was floating, flipping, and doing something like sommersaults, looked like he would spasm a bit all over the tank. He just died. I have never seen a fish die. Is this the behavior of a fish before it dies? Or is it the behavior of some kind of disease. I have a 75 gal. tank that is almost 2 years old. I checked my levels yesterday. Sp gr. 1.0231/2, pH 8.2, temp 78, Amm. 0, Nitrite 0, Nitrate less than 10, calcium 550, and alkalinity 4.5 meq/L. Anyone input would be great. I don't want something to effect the rest of my fish.
