Cloudy eyes are most common a symptom of recent trauma (e.g. netting the fish, or aggression from tank mates) or poor water quality. It is not usually a sign of ich.
I recommend doing a large water change using water that has been mixed at least a day. If that fails to improve the fish, then put the fish in a QT and treat with an antibiotic (e.g. Maracyn2 for sw).

Most commonly, it only affects a single fish. However, if this is a sign of poor water quality, multiple fish can get sick.
