Xenia? looks dead :(

Our xenia has been with us for only 2 weeks. It was doing fine. then we noticed the polyps or heads? turning brownish and falling off??? there appears to be new ones (white) coming out the arms where the heads have fallen off.. its just leaning and droopy and looks bad... before it was extending and waving everywhere. It was orginally in our main 55 gallon tank but we are moving our clowns into a 29 gallon and moved the xenia into that last night after finding a bristle worm under it.. did that cause the damage? it was looking droopy a few days before we moved it
all levels are good in both tanks too
any help to save it?

What's your water parameter numbers,found they like what they call dirty water. If your nitrates are high, I personally know mine doesn't like that to well.

We did a water test today and all the levels are fine
Nitrates were 20
PH was 8.0
Nitrites were zero
Alkalinity was at 300
The poor little thing has hardly any hands left now and looking really rough so not looking promising.
Any clues anybody?
