Yellow tang is sick :(

My yellow tang has a skin infection where it gets red blotches on its body. This is the second time its had it. I always treat it with antibacterial medicine. I noticed that my tangs face looks bony and has silver lines, as if the skin has been eaten away at certain spots. I read about this in a book and now know that it has a parasite... how do i treat this?
