Yellow Tang, Weird Behavior

So I had this Yellow Tang for approximately 7-8 months with no issues ever except Black ich when I first got him/her. So within the last couple days he has been acting a lot different. It appears he is interested in eating but won't eat. He will take on real small pieces of food but doesn't seem to be interested in food he usually loves.
With this behavior, he is also hiding in the rock work I have hovering in one spot. He will come out when he sees me, but briefly, and returns to his hiding spot. What gives? Nothing different in my tank except I recently changed the flow direction and replaced my media in my Calcium Reactor.
Last I checked my levels are good. Trates at near zero, PH 8.4, Calcium 420 (I think). This is a 120 tank that has been up and running for almost a year.

I would check for stray current (voltage) in the tank...Something may be breaking down and allowing some current..Do you have a grounding rod in the tank?

OK, woke up this morning and he's dead. So Weird. Out of nowhere. I can't figure it out. He was fine, and everything else in the tank appears to be doing well. I don't have a grounding rod either. Is it possible it was just its time to die, or was there something wrong with it?

Somthing was wrong with it. Any chance ANY drop of water from your blue tangs Qtine tank made it over to the main?? - a net/grabbers/heater/siphon hose/equipment - even a drop of Qtine water on a finger can introduce a parasite into the DT. Have u added ANYTHING to the DT from anyones tank in the last 30 days?

Hmmm. Interesting. A drop? Yes, very possible. Now what. I mean yeah, my hands which I thought were clean after being in the QT tank, or maybe a net. Is this a concern for everything else in the tank? Any precautions I should be taking now? Don't get me wrong I was not completely careless. I thought I would clean my hands or tools and thought it would have been all good. Holy crap I wonder if something like that did happen. I did not see anything on the Yellow Tang though. Everything looked normal

If you did have a parasite breakout in the main - the other fish will likely show signs soon, so I'd be ready to qtine&treat them.
I had a total effin disaster 2-3 weeks ago with new fish in Qtine. I was running 2 qtine tanks, one set up first 5 weeks prior and the second one running for only 2 weeks. The yello tang in the 2nd tank became very sick/week/ & crapped out everything he had inside him.....seeing this I freaked out and put him in the older running Qtine tank, which was NOT at 100% treatment for parasites, it was comming down - cause the fish were OK....
This tang died 12 hours after the transfer, and he had velvet...within 24 hours, the fish in the older running tank started dying. 36 hours after transfer I had lost all the fish in Qtine. I am still shocked and feel horrible for loosing 6 fish in 2 days, but they were new so there was not the emotional attachment I have with the ones I;ve had for many years.
WATCH your remaining fish, and be ready to Qtine....It is possible the tang just weireded out & died, but rather unlikely. If he did have somthing, the others will have to soon too. God i hate the parasites! they are so evil and easy to transfer from tank to tank..
