I have a 29gl tank and my phosphates are out of control. How do i get them down and out permantly.

I havew a 29 gallon tank and i am currently having problems with my phosphate levels. My nitrates are at 80ppm and my ph is at 7.8. Looking for a way to permantly remove the phosphates. Thanks
