All my fish are gone now...

Well, all my fish had died now. Everyone of them besides this little skinny damsal that I have been having for at least 9 months. All my fish are gone now and I have no idea what to do now. Anybody have any advice on what to do? Everytime my fish had died, it always had ick. I've remade my tank 1 time already which is 2 times altogether and both times, they have had ick. I have no idea what to do and need some advice. Thanks

Thanks for all the advice you have given me. Please teach me how to set up a Q/T which I have never done before. I have a 10 gallon right now and I was wondering if that would do. If that wouldn't, do you think a 20 gallon would work? What do I need inside the Q/tank. Please include everything that I need to keep my fish alive and healthy. Thanks
