Anenome dying with hole

I bought a bubble tip anenome last week and by fri-sat it made a hole right through the middle of itself through its feet and mouth and is dying I pressume, the green tenticles are still green and moving but very tiny. I use 2 Power glows and 2 Marine glows for my lighting (T5), Its an 85 gallon tank with Wet/Dry filter and protien skimmer, all water test are normal and all other fish and Inverts are fine.This has happened one other time last year, I dont know where to begin with this problem??

Very important to accilmate over 2 to 3 hr period, did the anemone wander at all? SOMETIME they shrivel up and you think they are dead or dieing and then the next day they make a miraculous come back, If you see a hole in the foot MOST LIKELY it will not make it, if anything starts to spew out of it REMOVE IMEDIATELY! and dispose, run some carbon, check you levels and do a water change as necessasary.

Yes I did a 3 hr Accilmation, and he did wonder from where i set him on the glass over to the rock then down to the bottom of the tank, then i started noticing the rock through the middle of itself as it sat on my live rock and knew that wasn't right
