Yes, the whole thing needs to be in the sump. I have seen pictures of a few modified units and ER is starting to make an external model.
BTW, I've been running a CS6-3 for 3 years now. There's been a few comparisons done on skimmers and for the most part the ER's are the most efficient. The PM skimmers (aka MyReef's) are probably a little better at skimming but the noise is a lot more and the pump size needs to be bigger. There is no skimmer that's easier to setup and maintain then the ER's. I had mine out of th ebox and skimming my tank in 10 minutes.
