Bloated manderian

Anyone know what's going on with my manderian? I just noticed it tonight and haven't tried anything yet. I've had this guy for over 4 years so would like to do what I can to get him better

Not sure. Could be organ failure or infection. What are the water conditions in the tank? What does the fish eat?

He just eats the pods in the tank but sometimes I notice him going after some pellets. They are formula one pellets.
