I have tried 2 times to keep this starfish and both have died within a week. I have a 125 reef with quality water testing. I have not direct feed and need some basic care instructions for this animal.

How are you acclimating?

My guess is one of two possibilities...
1) any chance that you have trace remains of copper in your tank? Was this a used tank, or have you ever used copper? Starfish have a zero tolerance to copper.
2) I'm more inclined to lean towards this....some inverts are extremely sensitive to variations in water, such as temp, ph, etc., and require much longer/slower acclimation times. I learned this with shrimp after going thru 4 or 5. Some would die within an hour, some a week later. But after I tripled my acclimation time and slowly adjusted them to the new water, no problems. Some people never have this issue and can just bring something home, float the bag for 10 mins., and drop them straight in. I have never been this lucky.

same problem... my bluie survived for about a week, till something chewed him up pretty good (thinkin it was the hermits)

It does amaze me though. I have witnessed my lfs get shipments in, cut the boxes open, throw the bags in the tanks, and literally before I even leave the store (maybe 15 mins.), they are cutting the bags open dropping the fish right in....Unbelievable. I did find out that they keep their sg lower than normal. Maybe this has something to do with it. That's not good long-term, but since they are temporarily housing them, maybe that's their justification.

I wonder how many fish really die at a pet store. I keep my sg around 1.025. I sting the bag in the tank and slowly add tank water untill I feel it is good.

Something I just thought of...If the LFS gets all their stuff from one supplier and they know what the supplier keeps their salinity at then they could try ro keep the store at that same salinity. It maybe explains the super lax acclimation time.

What are your tank parameters?:
A long acclimation is required. If dying in a week, it is due to acclimation shock...almost certainly, and the damage could have been done at the LFS. If your LFS specific gravity in invert tanks is less than 1.024 at a minimum, I would not buy them from there.
These stars are incredibly delicate and do need a very mature reef tank no younger than 6 months. They can not be spot fed, and really require a minimum of 200lbs of high quality LR in the tank (any in a sump is not counted). Surface area available for feeding is the key.
