i'm convinced it was dead before you got it!!! i've had dwarfs for years and they have been pretty damn hearty for me!!!! learn from this order!!! you said it about your yellow polyps :D

A CB is a great addition to Main. But to tell you the truth your better off getting one at your local LFS store. Like that you could get a better look your self.

I agree about purchasing livestock online. I don't trust someone else picking out my livestock for me and I feel much better when given the opportunity to observe/visualize the fish or coral.
Unless you are completely new to this hobby I doubt you wouldn't notice the CB if dead upon arrival. That does not mean it wasn't very sick.
And yes acclimation plays a major role in the immediate health of a fish once introduced. Some fish require 3 hours or more of acclimation. That is why SWF.com and most LFS give printed acclimation instructions with all livestock and recommend turning the lights out and not feeding for 24 hours once introduced. Sounds stupid but it goes a long way in minimizing the stress once introduced.
 
Sounds like one of those things you can't explain and you'll never really know what happened. I have a coral beauty that is doing wonderful. But I tried getting a flame angel years back and was so excited. After acclimation, I let him go in the tank and instantly hid in the rocks. That was the last time I seen him. The next day all I saw was his carcus! He died and immediately was eaten. A little hard bringing the white carcus back to the store for their "guarantee"! But I did and since we've purchased a lot in the past before, we got a credit. I think your coral beauty was sick or stressed before you even received it.

Sounds to me like he was sick when you got him. I highly doubt anything is wrong with your tank.
Awesome, btw, lots of rockwork in there!!

I spoke to the guy I got it from and he credited my acct. He said it does happen from time to time with Fedex depending on how they handled the box. If the box is flipped over, etc. it can cause major stress where the fish is doomed before you even get him. I want to get another one. Plus, I want a copperbanded and a hammer. So I may take a chance and get those from him too. I know. I probably shouldnt, but I might give the guy another shot

I would do some research on the copperband butterfly. I don't think they are reef safe.

The research I have done says they are reef safe...There is always a chance, but they are generally considered reef safe

I have had a 2 coral beautys before and they have all died these fish seem to be very sensitive mine did the same exact thing regarding the head stand. This is a good sign that they are unhealthy and they are on there way out this def a species of fish i would stay away from to sensative

FYI only.
Not second guessing you, paulcoates, but this site states, "The Copperband has an elongated mouth which it uses to pick at coral heads, including nuisance aiptasia." however under reef safe it says to monitor.

Not sure how you purchased your 2 CBs or when they were introduced but give them another shot. Dwarf angels are awesome additions to a tank as they have lots of personality and are typically outgoing fish. In my opinion, an established tank, 8 months or older, goes a long way in increasing the success rate in keeping dwarfs.

I have talked to a few people and read on a few websites that say they are largely consider reef safe, but to "monitor" them anyway

My understanding is that the copperband butterfly isn't really big on eating the coral, but it is the little inverts on your live rock that you have to monitor. Tube worms (feather dusters too?), and the various other creatures that live in you tank. Mine didn't mess with anything other than my tube worms. Not to say that every fish will behave, but mine did. Then again, I only had mine for a few weeks before he contracted brooklynella, and my whole tank was wiped out because of it. But he behaved while he was still alive.
