In any event, there are a few problems I can see...you have a trickle filter that you clean every couple of months. You need to clean any media more often than that, sometimes eve as much as once a week, but your bioload doesnt look too bad, so every couple of weeks may be fine. And more than likely do more frequent water changes.
What source water are you using? Have you tested phosphates at all?
I also agree, you probably need a lot more circulation in the tank.
And how long do you leave your lights on?
