Dogface Puffer Help!!!

Hello There Guys:
I hope that somebody out there will be able to help me!
I've had a dogface puffer for several months now in my 90 gallon tank. The past couple of days he has been "sulking" at the bottom of the tank with his tail curled in. He looks very lonely and depressed and often times pulls his fin inside of his body where his gill is. Every now and then he will swim to the surface and swim in place, but still with his tail curled in.
At first I thought it was just his behavior and will pass, but it is yet to change. He also seems to "scratching" himself off of the live rock along and among the sand bed. Also, he seems to be swimming in a very "disoriented" manner (bumping into the glass and rocks, etc.) However, he is still eating normally (fresh squid and frozen krill).
I've noticed tonight, that he seems to have little tiny white spots on his fins (which I'm now assuming could be Ich). However, it's hard to tell if they are indeed spots or just sand, due to him rubbing and laying on the sand.
In my tank, I have 1 Dogface Puffer, 1 Niger Trigger, and 2 Percula Clowns along with several crabs and conchs. They all get along wonderfully and have no problems at all....except my puffer.
I've attached a photo of the guy. Beware, that the picture isn't the greatest, due to the fact that it was taken on with my phone.
Please help! I'm really worried about this guy!

How long have you had this fish?
When you say perfect, what do you mean? What do you keep your salt level? What are your nitrates?

PH = 8.2
AMMONIA = 0ppm
NITRITE = 0ppm
NITRATE = 0 -5.0 ppm (hard to specificaly accurate)
SALINITY = 1.024
I must have had this guy for well over 3 months now. I just noticed 5 minutes ago, that it looked like he was "coughing" or trying to "hack"" something up. Is it possible that he could of swallowed something that got lodged? I do have some snails in there for treats to keep his teeth filed down.

What is he eating? Hard shelled foods need to be included to keep the puffer's teeth filed down.
You need to confirm the ich so you can start thinking about treatment. Use a magnifying glass to check him over.

I feed him squid and krill everyday. However, I do also give him plenty of snails and shellfish for the sole purpose of keeping his teeth filed down. I can't judge how many snails he eats a day...but it's probably at least 6. I put in about 50 snails every couple of weeks and he searches and destroys them at his own leisure. As he eats them, I remove the empty shells and pieces from the tank.
I read your articles on fish diseases/ich and it was extremely helpful! Thank you so much for that information!
If it is indeed ich, what is a proper medication to use? Since he is scaleless, I know that I can't use a copper based mediacation.
Last night I slowly started to reduce the salinity in the tank, like you recommended. It was 1.024 and now it's reading at 1.022. Over the next 48 hours I will continue to slowly reduce it.
Would a fresh water dip be helpful or just stress him out even more?

You need to do hyposalinity for ich. You'll need to treat all fish exposed. The procedure is outlined in the FAQ Topic at the top of this webpage.

We are having the same problems with our dogface. This is our second guy and both ended up with ick. We are about to treat for with hypo. None of our other fish show any signs of ick or stress but for some reason it seems like both our dogs got it fast and hard. You sould start treating with hypo imediatly. It's hard to bring them back once they go to far.
