Fish keep dying

So i have a 90 gallon with about 40 lbs of live rock and 80 lbs of live sand. I have a fluval 405 and a powerhead. The tank cycled for about 2 months before I put any fish in and the water levels were fine. Over the course of the next 4 months I added 1 yellow tang, 2 percula clowns, a cleaner shrimp, and have 15 turbo snails.
everything has been working great for 6 months. About 3 weeks ago I added a Heniochus which died after a week. Last week I added a naso tang which died after a week.
Do I have a compatability problem? All the fish that I added were eating well and showed no signs of sickness, then one day, dead. Very sad and disappointing. I can't find the problem! Help! :notsure:

i generally take the bag that the LFS puts the fish in and place it in the tank so the fish can get acclimated to the temperature for about an hour. Then I slowly drip in some water from the tank and let them sit for about an hour and then let them loose.
