Hammer dying?

This is a pic of my hammer at night closed up. The heads in the middle appear dead, and there are 3 now that look like that. My parameters are all excellent. I dose b-ionic 2-part every day or every other day and test for mag, calc, alk, and pH. I have 2 monti caps and an encrusting monti doing very well and growing quickly. Same with frogspawn, candy canes, gsp, xenia and, zoos. Half of the hammer is not connected to the dying part and opens up huge, like it all used to.
In the past, sometimes the head appearred dead for a couple days while it was splitting, and 2 heads would come back but this time Im worried.
Is there a disease that can kill off the heads?
No other corals are exhibiting any problems.

My frog did the same thing. I had 3 heads on it when I bought it, but now I have one. I have had it for about a year now and the one head is doing fine. I noticed it has 2 mouths on it now even. it has doubled in size too.

It didnt get damaged at all. It is epoxied to the LR. It seems like it has spread from the first head to the other 2 that are connected.
Ill just hope for the best.
