Help placing Hammer, exposed skeleton.

Thanks for reading. Bout 2 weeks ago I moved my hammer which was doing great in it's original spot.
I purchased a frogspawn to put in his spot and move the hammer up a little. Since then, it's been showing a lot of skeleton.
Did the hammer move itself all the way to the other side of its skeleton? Will the rest of the him go back or grow back? Should I move him again back to his original spot? I have a pic of the current placement, and a pic of the hammer by itself. Thanks again for all your help.

I think you should move it. I would think it is pulling off because of too high of flow. Does that seen right?

Actually, it was getting more flow in the middle. Im really confused.
