Help! Spiny Box Puffer white spots

Help! We've had a spiny box puffer for a few weeks. His behavior and appetite is fine and normal, but he has developed white spots all over, fins, body, etc. At first I thought it was Ich, but no other fish is affected (we have 3 clowns, a coral beauty, and 4 blue damsels). As a precaution, we've done Ich treatment and done all the water changes, etc. Salinity and all chemistry within normal limits.

The spots seemed to go away, hut now they are back. My Google search has turned up nothing.

Please help! Any info will be helpful!
