It is very possible if you do not do a well-enough observation while the fish is in QT. If the fish carries one spot of ich into the tank, it surely could show up 3 months later if the water quality declines drastically, leaving the fish open to attack from a waiting and anxious parasite, hanging on to live to that point.

I QT'd the last fish becuz it was a tang and I knew their reputation for ich. I did not QT prior fish. So the ich could have been present on one of the prior additions (meaning ich had been there 6+ months without me knowing it), it could have been on the tang in QT and I did not see it, or ich got into the system on something else like LR, coral or inverts.

A QT, cheap NO lights and a magnifying glass will go a long way in detecting ich.
Hobbyists you look at their fish frequently thru a magnifying glass will be way ahead in discovering any problem, and, therefore be able to address the problem sooner.

Since I put all my fish in hypo, I haven't had ich present in my main tank yet but there's always that chance that it could somehow get into my system.

you can do hypo but if your fish have ich and if you ask me for 6 weeks and when you do hypo always check 2 important parameter (the noraml to) your ph level and the salinity level every 2 days if your ph will drop the hypo will crash and if the salinity will raise (by lose water) you will get ich free again and the treatment will need to extend again for 4 weeks . the regular maintain need to be to but those 2 need to be more careful to get a good result with the hypo treatment.
