Welcome to the site. sorry about the losses.
The spots from ich only SEEM to go away, actually it's a stage of the parasite to drop off and be free floating, so you were never rid of it and it wasn't "gone".
Ich is a parasite and until you get rid of them, your tank is never safe to add new fish. New fish are stressed from the move, that makes them more susceptible to the parasite, and once infected, by sheer numbers even the healthy fish who had managed to not be overcome, can be overtaken as well...killing every fish in the tank.
Inverts are not affected by the parasite, only fish. To get rid of ich,
the tank has to be left FISHLESS for 8 weeks. That means removing every fish in the display tank and treating them in a hospital tank. By quarantining all new arrivals, you have a much better chance of preventing the parasite from being introduced in the first place.
For the healthy fish...a cleaner shrimp likes to eat the parasite from their gills and they even set up a little "cleaning station" where the fish will come to just for relief. Feeding fresh garlic juice soaked food will also help immunity. However as soon as any stress happens, an new outbreak will occur. New fish don't have a chance because of the stress level they suffer already.
