Internal parasite?

I have a pair of false percs, I put them through a 3wks QT, no sign of ick or any type of external parasites one of the percs semed a bit thin, but I thought it was just stressed out due to being in QT, they have been in my display for about 3wks, one of the percs looks fantastic, the other one is just getting thinner and thinner every day and it eats like a maniac. I am thinking it must be internal parasites, if so, what would the treatment be?
tank specs:
58gal,with fuge/sump.
Ammonia-0
Nitrite-0
Nitrate-close to 0
pH-8.2
alk-3.0
Ca-400

Bumping this up: this poor fish is not getting any better, eating like a pig not gaining any weight, getting skinner and skinner.
Any help would be apprieciated.

Thanks Terry- He eats formula one flake from my hand every day, then he gets fed with the rest of the tank every other to every third day, I rotate mysis, brine, and my own concoction of mixed sea foods, and he grazes off the nori sheets I put in once a week. I really cannot figure out what it could be other than internal parasite, Oh and this is an, aquacultured fish. It's color is not fantastic, but there are no lesions, or sign of external parasites at all, he acts fine, just thin.
I keep my salinity at 35ppt.

I will try to be a bit more specific:
1. There are three fish in the tank, two false percs (out of QT the first of March) and one clown gobie (out of QT this week)
2. They are in a 58gal reef tank with various corals and inverts.
Filtration is a sump/fuge with protein skimmer, and around 90lbs of LR. Lighting is one 250w MH.
3. Salinity is measure with a refractometer. is kept at 35ppt, Ammonia 0, Nitrite 0, nitrate 0 alk 3.0 Ca 400.
All other fish in tank are fat and happy, This fish did look a bit thin and pale from the beginning, but I could see no sign of external parasites (I have delt with ich, and gill flukes before), or anything really, it ate well and seemed okay. I though it just needed to get into my display where it could feed all day off the LR and be in a more secure invoironment. It has been in my main tank about 3wks. I wouldn't say it is loosing anymore weight, but it is not gaining (I have increased it's feedings in the past few days) it's respirations are normal and behavior is normal. The only thing wrong is the color is a bit pale and it is thin. I have not been soaking food in garlic but I will certainly start doing that today. I am not familiar with piperzine, how is this treatment done.
THank you for all your help. I do understand it is hard to diagnose something online and I really commend you for the time you spend trying to help us. I worked for years in the animal field and it is hard enough to diagnose stuff when you are right there looking at the animal.

I have a false perc with similar symptoms. Pale and thin when I first got him. He initially didn't eat much. The bigger perc always outcompeted him for food, but I started soaking the food in Garlic Xtreme so he finally got the picture and started eating. Two weeks later his coloration is slowly coming back. The edges of his fins are now dark black and he's gone from yellow to a mix between yellow and orange. He's also a lot less transparent looking.
My guess is that he was the runt of the litter and since he was held in a tank with multiple percs, he was underfed. Maybe yours went through the same thing?
Another thing you can try, which was suggested to me by Bang Guy is feed Cyclop-eeze. The cool thing about feeding this is that the percs go nuts over it and nothing else in my tank is interested so they don't have to compete for it. The stuff I got was Liquid Life Marine Plankton. Buy it from an LFS since you'd have to pay an insane amount of shipping online since it's frozen.
But now you've got me worried about internal parasites. Yikes. Is this something the fish can kick themselves if they're healthy and eating or is medication necessary?
