Did that develop over time, suddenly appear?? Is the QT well cycled?

My qt is actually my 16 gallon bowfront that has been set up for a year now. Have the fish for about a month and just noticed this recently. Only thing i've done is treat with prazipro for flukes.
Wondering if this is some type of parasite? Or maybe bacterial infection?
Only other tank mate is a radiant wrasse.
What do you think it is, will try to get better pic.
I'm hoping its not contagious.

It does look like a mouth infection to me. You can treat him with Maracyn 2 for SW fish. Be sure to have plenty of flow in the QT.

Here's an update, i did a big water change. Next day the wrasses mouth looks better. But it looks like something is eating away at his skin, i can see redish sores. Could it be flukes?
Now my radiant wrasse has something near his mouth, i can easily see inside the mouth there is somthing growing also.
Fish are acting normally, eatting well no scratching.
I'm going to get a seperate tank ready to treat. I'm also going to dip in prazipro for flukes.
