Is this Toadstool dying or dead?

Yea, I have moved it several times. A week in this spot, then a week in that spot. It was on a rock for 3 weeks until I moved it to the sand.
As far as water quality, I am not convinced.
I test my water every week. I change 15% every other week.
After cycling...
I have never had an Ammonia reading, Nitrite reading, Phosphate reading or Nitrate reading.
My dKH has been between 7 and 9.
My pH has always been between 7.8 and 8.4 so that is a little varied.
My calcium has been between 380 and 420.
I switched from tap to DI water. I had a hair algae proble, now I do not have any algae.
I guess I don't know what from the water quality perspective I am not testing that could throw off this coral and only this coral.

What kind of flow is the coral in????
Leathers like high light (though they are adaptable) and fairly strong, alternating currents. My GPT's grow like crazy with intermittent flow and will close up over time if they do not have enough. Yours looks like it is collecting debri and in trouble - I think you need better flow on this guy.
