Filterfeeding clams can bury themselves in rocks and substrates... but what might have happened is that what you thought was a rock was actually part of the clams shell... and then it started to move further up into that piece of rock. That's just a theory though - and may be way off.
Peanut worms shouldn't have any effect on rocks - but I haven't researched them in a long time.

To put it in perspective.. that hammer coral is about 4 inches across... (a lovely $15 addition to my tank from SWF.COM) He has GROWN a lot in three weeks....
I still cant believe how big this thing is... haha
