Ok... you've had some great advice already, but let me throw some more out there.
Do a thorough search for the word "cycle" on this forum. From your response it sounds like you might be a bit confused as to what that means. Briefly, you "cycle" a tank to allow bacteria to grow to the point of maintaining the nithrogen cycle. Bacteria needs food to survive.. if your tank has been fallow for months then your bacteria was dead.
ALWAYS test for ammonia after adding fish. Ammonia is toxic... the presence of ammonia is a sure way to know your tank is in trouble.
Sand on top of CC is bad. You need to go with one or the other. Mixing the two makes it impossible to clean either properly.
Medicating your display tank can be really bad.
You mentioned a QT tank so I'm not sure how the ich got into your display tank. Have you tracked that down yet?
