My critter is dying

I can't report my dying tree sponge because it was an add on item and does not appear in your list. I tried to report it anyway and and an error message pops up an says I have to select an item to report...AAHHH
As you can see by the pictures it is dying...Please advise.

You can do it. You just select something /anything on the form, to activate it and then just attach the picture and add a note in the comment section that the sponge was an add on to the order.
