no luck - 3 dead, 2 alive and 3 diseases!

ok here is how it went down:
first i ordered a flame angel and a CBBF from this site.
The CBBF was DOA (1 down.) the flame was ok.
went to the LFS and picked up a new CBBF. within a week ich started showing up and i began the hypo treatment in my 29g tall QT. they kept getting worse, and both died (3 down.).
i re-evaluated the QT situation and made some changes. the UV has been hooked up to it for a month, it is basically bare bottom now, with no sand or porous places for the ich to thrive. made some water changes, time to try it again.
enter the latest fish from the LFS. new flame and third CBBF.
without hesitation, i began the Hypo treatment almost immediately. after 3 days a few signs of ick show up, disappear, and have not since returned. the hypo is working. i keep an eye on the levels. ph dropped a little bit down to 8.0, the trates were at 5ppm, and the Alk was at zero!

so i do a few small water changes, to bring the trates down, vaccuum out any food and gunk on the bottom. and aid with the pH / alk issue. i also dosed with red sea marine success buff for pH / Alk
3 days later, last night, i check again , pH still 8.0, alk rising slightly, i dose again.
but now, the CBBF has maybe 5 showing spots of black ich in total, on pec and tail fins. it seems the best cure for that is formalin baths, but the LFS cant sell it. they thought there are legal issues with it. so i have to order it online, unless someone has a better idea.
also,the flame was showing minimal interest in food, and the onset of what i can only believe to be HLLE. small pits behind both eyes.
so i replaced the very old heater with a new one, in case it was causing stray voltage, and dosed the QT with vitamin C. he ate some food a little later. other than that what else can i do???
this morning they are both 'breathing' very hard ! why?? this is getting very frustrating, i dont know if they will last the day!?!
its like somebody up there really doesnt want me to have these fish!!

How do you have your QT set up, and what are all your readings?
What are you using to measure salinity?

omfg - i just killed the fish..... :'(
as i mentioned i replaced the heater. it wasnt a very big one, but i guess its super powerful, so for some reason i set it at 81 the night before, and when i came home last night, the CBBF was dead and the flame was in real bad shape. i saw the temp and it was 87!!! i figure thats why they were all of a sudden breathing so hard in the morning and dieing/ dead now. i immediately took the heater out, and added some chilled RO to reduce the temp... this morning the temp was down to 80, and the flame was still hanging on, but not looking great still. i would be very surprised if he pulls through.

turbo twist UV, with a rio 50, water trickles out.
maxijet 600 for flow.
penguin biowheel for filtration - not sure of size
NO or HO flourescent lighting - basic light bar
bare bottom, but i placed some plastic sheets down so there is no reflection
fake corals for hiding spots
pH is now 8.2
alk is now 1.5ish on the red sea test kit scale, which is in the avg-low range.
ammonnia trace
nitrite none
trate <5 ppm
i checked phosphate , 1, which isnt great but , the instructions said it does nothing to fish.
i was doing hypo
so sal = 1.009 - 1.010
(refracto)
i think everything is right at this point, and i was stupid about changing equipment, while being just plain unlucky with the black ich and the HLLE ? he might have been irritated by some remaining copper in the system, even though i had done some significant water changes since then...
