Old fish in new QT?

I have had a big fish wipeout due to ich for the past 8 weeks. One by one, 7 of my 9 fish died.
Remaining: 1 tomato clown, 1 sand perch goby
The sandperch has been itching a little, but neither one shows visible sign of ich. Last death to ich was 1 week ago.
Question: should I transfer both fish to my newly set-up QT?
If yes, how do I treat them - 10 gallon tank set up with 50% old tank water and old tank thin layer of sand and 1/4 lb. of live rock from old tank as well+ some new fake rocks, fake plants, etc... for hiding spots.
I have Formaline with malachite green - how much do I dose, how often?
Do I administer hypo-salinity as well through the QT period?

Do not does the qt with those meds, as it would be a waste of time, and contaminate your QT.
Are you saying that your QT is not cycled? How did you set it up?
Take a look at the post in the FAQ Thread at the top of this forum, and the post there on hyposalinity.

what is wrong with the medicine?
I have seen my LFS use it before so I thought it would be ok.
I set up the QT with about a cup of sand from my old tank and 50% water from the old tank as well - hoping it wouldn't need to be cycled.
The clown is there right now.
Let me know what you think I should do.
