picaso trigger

i have a picasso trigger that has started to get popped out eyeballs and is scratching on the rocks causing marks behind his eyes can you tell me what this is and how to fix it?

Sounds like beginning of ich. What is in your tank right now? Do you have a quarantine tank?

no i don't have a qt but i am working on getting one. i have a trigger puffer and a clown along with some inverts. however only the trigger is showing these signs.

Well regardless of who is showing the signs, all could potentially have or get ich because it is in your tank. This is why all fish would need to be quarantined and put through hyposalinity.
What are your water readings? (ammonia, nitrite, nitrate, pH, kH, temperature, salinity)

ammonia is <.25ppm nitrites are <.5ppm and nitrates are a little high 15ppm ph is 8.0-8.2 temp is 73 salinity is 1.021
