Possible anchor worm on Tang

Hey all, I think my Kole Tang has a parasitic worm. Looked through some threads and I think thats what it is. I can't get a pic cause he's so quick and goes hiding into rocks. So heres the story, it looks like like a tubular off whitish "sponge" that is attatched in the area between where their fin attatches to their body. It's only one side. It looks like it has eyes. I noticed it this morning. Have had this tang for over a year. Here's my tank specs.
125gal. with sump/fug. 40gal ; skimmer
200 lbs LR
tank established for 5+ years
nitrates 0
nitrites 0
amm. 0
ph 8.2
temp 80
spec grav. 1.25
did 20% H2O changes yesterday (didn't see spot there yesterday)
So the only thing that I can think is that on Saturday I introduced some new zoo's and a clam into the tank and a fighting conch. No new fish introduced.
I know its hard to explain without a pic. I tried to catch him to put him in QT, but the bugger is so fast. Also I have a sixline wrasse in QT who's 4 weeks won't be up till the end of Sept. Here's my quest in a nutshell:
1. Is it a worm or something else?
2. Contagious to other fish in tank?
3. Ho do I catch him?
4. Should I put him in QT with sixline?
5. How the heck did this develope?
Also he's eating like a pig, I prov. garlic and vits. zoe and zoecon into their food, they get fed once a day.

ok here's an "artist rendering" of what it kinda looks like.
I'll try to get a pic of him tommorow if possible.
