What kind of equipment do I need to start a QT tank? I'd like to set one up but i'm not sure what to put in it. Do I need sand, rock?

IMO rock is allways a good thing to have other than that how big is the QT tank. I would not get to crazy on the filtration. May be a hob type of filter will do just fine. QT tanks are only temporary. Just change the water regulaly and you will be just fine.

Get a hang on filter for the QT tank. Take out the foam and put it in your refugium of your display. Now it will get all the bacteria needed built up so when you do setup the qt, take out the sponge and put it in the filter.
No live rock or sand, use plastic fake rocks from LFS or pvc fittings. You may need a treatment, and having live rock and sand is not a good thing with many meds.
Heater, thermometer and power heads are also needed.
Keep it simple

Some people don't realize this, but putting the foam in the dt isn't necessary. Just leaving it in the qt and ghost feeding will eventually culture the necessary bacteria. However, I do recommend putting it in the dt sump because this will definitely help the bacteria culture a lot faster. Excellent suggestion
Like he said, the rocks and sand will obsorb medications and affect their effect on your fish. Keep it simple, keep it bare bottom. Keep it real and good luck.
Addicted4life? I haven't seen you around here much. I too am from Indiana. I live in Muncie though, so no good fish stores here. What's you favorite fish store in Indy? I'll have to stop buying online and actually go to a store.
