Red spot(s) on tang and PH issue in QT

My Yellow Tang is on a QT undergoing hyposalinity for a minor case of ich. She has been in there about 3 weeks now. I have now noticed red splotches almost under the skin near her tail region (near the scalpal). I did add some copper in the begining to the water but did not replace once doing the hyposalinity. My nitrites are through the roof and I have been doing frequent water changes to correct this. Also, my PH seems to be low (around 7.8-8.0-in between colors) because of the hyposalinity. Any suggestions on helping that as well?

Well, the nitrites are a big problem. For low pH, simply adjust using a buffer, but make gradual changes, not major changes all at once.
If you have not seen ich in 3 weeks, you can go ahead and slow raise the salinity and begine to acclimate the fish to normal saline conditions. The process of changing water should reduce nitrite.
See if this helps. Start doing some heavy garlic food feedings.

Well, despite all my efforts, she passed this morning

I honestly tried everything I could. I feel so guilty.
