Are you supposed to leave the plastic syringe tip on when your adding the reagent?

Yes.

Direction wise, it's not an easy test to understand at first, but after you do it you'll be like..... ooooooooooooh

Yes. You'll have some reagent in it. If you were to take it off you'd get a very inaccurate reading.
