You have 30 or 60 days. No, I need to send you the form with my portion as the sender filled out. YOu also need the insurance slip which I have. Once you receive this from me, you take the damaged filtr alnog with the original shipping box and xontents to any USPO brance with the form and slip I send you. You will have to fill out your portinon on the form. I will mark my portion so you receive the check.

Yes.....I have filled the form out. I sell stuff sometimes on ---- so I already had a form here. IT is filled out and ready to go. You need to sign and fill out your portion as the adressee. I have marked it so you get the check.
