Shrimp keep dying and Green Eye Polyps Decreasing

Hello,
I've had my tank setup for about 5 years now and recenlty lost my Cleaner shimp and about 4 months later my fire shirmp. My cleaner shimp was about 4 years old so I thought nothing of it and my fire shrimp was about 2-3 years old so again never realy thought anything of it.
Well.... I ordered a new cleaner shrimp, lettuce nuttibranch, and 5 peppermint shrimp. Well after about 1 1/2 months, I only have 1 peppermint shrimp left. Something is amiss in the tank and I just cannot figure it out.
All the parms are great.

The only other thing I've been noticing over the last 4-5 months is that my green eye polyps are decreasing, they use to be growing, but now have been decreasing weekly to just about nothing. I do have some brown eye polyps and those guys are doing great. I have different types of mushrooms and while some oare splitting a replicating, some are decreasing (different types).
Any thoughts to why I cannot keep shrimp and why my Green Eye'd polyps are decreasing?

Are you dosing anything?
How often do you do water changes? And what percent do you change?
Are you using tap water? If so have you looked at your EPA report for the copper, zinc, and tin levels?
What is the Alkalinity level? (for the Zoa)

I've been using RO/DI water for about 4 years now. ALthough, my TDS meter says the RODI water is messuring at 25 seems high to me, but better then my tap water at 398 :)
Doing 20% water changes every 2 weeks.
Only had a quickdip test left for alk and that measures IDEAL (300)
Calc is around 330.

Try testing for chlorine. Your di resin might be leaching. 25ppm is a concern. You never know what it's letting through.
