Sick Blonde Naso Tang

I have been having challenges with this Blonde Naso. I learned my lesson the hard way without a quarantine tank. I placed him in my display tank and a few days later he had black ick. Soon three other fish had white ick. I have been dealing with one at a time. I did a formalin dip every other day for 4 treatments and he seemed better. I did not have a quarantine tank so since it is a fish only tank I took the live rock and half of the sand out and did hyposalinity. I am at 6 weeks in Hypo and ready to raise the salinity and now the Blonde has this spot on his side I am guessing it is a bacterial infection. I started Maracyn-two last night but wanted to see if anyone could help me Identify this. Today it looks like little white worms inside...

The tank parameters are: Salinity 1.009, PH 8.0, Ammonia .25, Nitrate 5.0, and Nitrite .25
Thanks!

The naso is being harassed. What you are seeing looks like the site where the fish is being picked at. You can see the emperor picking on him in the vid.
You'll need to separate the fish.
What did you use to measure salinity during the hypo? Did the ammonia and nitrite spike when you removed the rock. Medicating a tank that will house live rock is always a very bad idea.

Thanks Beth! I cannot believe I did not notice the angel doing that. I put a divider in my tank until I can get the new quarantine tank established. Do you think they will they ever be able to be in the same tank together?
I purchased a refractometer to measure the salinity. Yes I did get a spike when first removing rock but I had it under control until this week the nitrites went up from 0. I plan to continue the Maracyn two for the remaining three treatments since I have a puffer that has not been eating.
Of course LFS sold me the Maracyn and said it was safe for live rock. So I actually did the rounds of treatment when the live rock was in 7 weeks ago. As soon as the final treatment of Maracyn is done I plan to raise the salinity and hopefully put the rock back in. Do you think I should add a bottle of the bacteria after that?
Thanks again!

I would not dose with the antibiotic anymore. It is killing off your bio-filter. Will not be able to attempt adding LR back in until you can significantly dilute out the Maracyn. I would work at doing 100% water change.
Doubtful that the fish harassing will stop. What is the size of your tank?

The Naso did not make it .. it is a 90 gallon. Is this common of emperors? I also lost an anthias that looked the same so now I wonder if he met the same fate. Now i am worried the Emporer will kill off others in the tank. Thanks for the advice I will stop the Maracyn. And do a water change later today.

Any fish can become aggressive. They do not necessarily pick on all the fish. Do several water changes over the next week or so. If you can add a carbon filter, do that as well, changing the carbon every other day. Then, add the LR back.
After this, its time to set up a quarantine for future use.
