Sick Scooter Blenny

My friends scooter blenny has been hiding and not coming out for food.
For six months the scooter blenny was zipping all over her tank.
Her tank is 28g:
45lbs LR
2 inches of LS
2 false percs
1 green clown goby
1 decorator crab
1 emerald crab
cuc hermits and snails
Assorted soft corals
Water checks out fine.
Approximately 3 days ago her very active blenny disappeared. Yesterday we tore the tank apart and found him hiding in the sand bed. I gave him a FWD and did see what appeared to be tiny white parasitic worms fall off of the blenny. I also noticed that the blenny was excreting a large amount slime. we placed the blenny back into the DT and it disapeared again.
We we caught him he was active and showed no signs of infection, but what would cause the blenny to suddenly dissapear and not come out for feeding?
I thinking we should put the blenny into a QT, but that would me dismantling the tank again.
Input please

i dont know exactly what could be causing the white worms or excess slime production, but i think you should get the fish into a qt. you're going to have to tear the tank down when the fish dies, which it will if it doesnt get treated, so you might as well tear it down and try to save the fish.
