sorry guys for the bad luck factor.. and not to steal your post, but what does setting up a qt tank from scratch consist of? I hear often a 10 gallon will work, but with what filteration etc?
and I am assuming the water has to cycle forehand?

i have seen ich on a mandarin before...... so i would remove the mandarin as well...
i know the mandarin has a really thick slime coating which makes it impossible for ich to attach itself, but what about its gills?
so definitely remove the mandarin as well.
was he NOT removed the first time?
that might have been your problem... that or the 4 and a half weeks only.

Check out the Common Treatments FAQ at the top of disease and treatment for instructions on setting up a qt tank.
