Yep, tap water,foods, but also some salts or additives may contribute.
Process of elimination needs to start at t he beginning.
First check your fresh water.
Then check the water after you've added your salt.
If those are phos-free, then add food and check.... so on....

Don't forget that many brands of activated carbon not designed for saltwater (and even some that are) are "activated" during the manufacting process via hydrochloric acid and other acids. This leads to phosphates.
