What type of lighting do you have? Try feeding it once a week or maybe twice something meaty. Some mysis or krill of cyclopeeze. Put the food on it's tentacles close to the mouth. It's color should be back to a tan shade to indicate that you are getting it back to normal.

Yup hairy mushroom coral. Medium light requirements. Photosynthesis and will eat smaller meaty foods. Aggressive to other coral and will propagate very easily, too easy some will say. Have seen them take over a tank.
