I don't have a refugium or a sump. I only have a 29G. But my flowerpot seems to be doing ok. But all my levels are 0 and I dose with many chemicals on a weekly schedule. I've had a few months. We'll see how long it last. Won't be the first thing I've bought that didn't make it. lol
dosing without monitoring can cause water chemistry to go out of wack. water change is often enough to replenish any lost trace elements.

I've noticed brown slime stuff go all around it. Also, the base is turning yellow, and it looks like it's dying...

Rewind!
Bring it to the surface. Lift a little more then smell it. If it smaells like it died a hundred years ago, plus this description, it would be brown jelly disease.
Let's rule that out first. If you have that, the coral needs to come out.

I am not an expert, far from it, but I see you have it on the bottom in the sand. I had mine for over 5 years before I sold my tank and it did real well. My flowerpot loved the current, I had it mid-high just under my Emporior filter and it thrived. I found it loved the light also. His arms (as I called them) would just flow on the current and it stayed open all day, try moving it up off the sand by a current and I bet he will do better for you, will also help get rid of the BJS. Also try to get off as much of the BJ as you can, an iodine dip can also help. Some say treat w/ Vitamin C and that should help. Some how it got an injury, mabe a snail or shrimp brushed it, anything could have happened to it.
Here are a few pics of mine so you can see where it liked to be... Good luck w/ it.
