Hlle??

I have had a sailfin tang for about 2.5 years. It was originally in my 55, where I ran carbon 24/7. It started showing signs of HLLE on both sides of its head. I stopped the carbon and then I upgraded to a 180. One side healed completely and the other side healed almost completely, but now it seems as if the one side is getting worse again.
I haven't changed anything maintenance wise except vodka dosing for the past 45 days. And I changed my light to a more powerful one.
So I was just curious on whether they had any new findings on causes & treatment.

What is your nitrates?
What type of tank set up do you have, filters, skimmer?
Food offerred fish, and frequency?

Nitrates hover aroun 50, hence the vodka dosing.
180 with 30 gallon sump, 225lbs LR, 150lbs LS, ASM G3 skimmer. Feed Formula 2 frozen-once-a-day; Mysis frozen or brine frozen-once-a-day; Aglae Sheets-once, sometimes twice a day, but always grazing on algae in the tank; And very occasionally pellets or Formula 2 flakes during the day before the main feeding in the evening.

I would add a much bigger sump with a bigger fuge. I run a 30 gallon on my 75. I would imagine a 55 would work best. That way you could probably have a 30 gallon fuge all by itself in there.

The nitrates is the issue. Why do you have them so high? Do you use a canister? Do have fine sand, or course sand, or crushed coral? Do you have a wet-dry system?
I would suggest adding a refuguium as well. What do you use for circulating water?
Instead of doing large maintenance water changes, try changing out 2 gals of water a day, in addition to top off water.
Use zoecon on food.

I'd love to have a bigger sump...I just don't have the room for one.

I'm not trying to start an argument, but I have seen FO tanks that have nitrates in the 100-200 range with no HLLE, so why would that have an effect? I do not have a canister, I have medium grain sand, the stuff that comes in a bag from the drs, and no wet dry system.
I unfortunately don't have room for a refugium and the macro would die anyway with the vodka dosing as my nitrates will be zero once I get to my maintenance dose. For water circulation I have 2 Eheim 1262 Return pumps and 2 MP40w Vortechs...No problems with circulation!!
Again, changing out 2 gallons of water a day would completely defeat the purpose of the vodka dosing and not allow me to find out when I have reached the sweet spot.

My LFS has a 1000 gallon reef with 0 nitrates yet a few of the purple tangs started to get hlle. I would love to see a study done on hlle. I've tried everything for my hippo tang yet I see more and more exposed flesh on his face. So will a fish end up perishing from hlle ?

From what I understand....no, a fish can live a full, long life will HLLE.
