With the situation you have, IF I had to use a bottled medication I would use Kick-Ich. When I first came into the hobby I used the product before I knew better treatment options and relied on advice from my LFS. It should help decrease the parasitic load and with the inverts you described be 'safe' for them.
Cleaner shrimp: Hit or miss. They will set up cleaning stations and if the fish allow them to clean will do a job at reducing the parasitic burden and remove any dead tissue as well.
Garlic: I would use fresh garlic, if any. The theoretical active ingredient degrades quickly...so IMHO bottled preps of garlic are worthless. I have commented on garlic before (forgive me Joe and Sep about the soap box I am about to climb up on)...There are no scientific, peer reviewed studies that have show that garlic is immune boosting in mammals or aquatic organisms.
If you find one I would like to read it as I will be able to scientifically justify why I have used it at times. It may help, it may not. It does not appear to hinder. Do I use it at times..yes...why
