It really appears to be a pretty bad case of HLLE (head and lateral line erosion) The diet is not the best for this fish, being primarily a herbivore. The condition can sometimes reverse itself but it's a slow process. I would add regular offerings of greens to the diet; nori, sea veggies, spirulina flake. Stray voltage is often considered a contributing factor but my guess is the diet insufficiency.
+1...I know cause I had the beginnings of HLLE in 3 tangs...I added algae sheets, formula 2 cubes, selcon, zoe, zoecon (vitamins) Garlic, BETTER FOOD, and I purchased a titanium ground probe....2 out of 3 of my tangs that had it are cured...one is much better, and yes it is a slow process either way.
