MANY people use base rock or "dead" rock and then place live nicer looking rock on top. This works great and in no time at the entire base will be seeded by the live and become live itself.
As for making rock that’s up to you. It works great and IMO is easy to do. All you need is some artistic ability, Portland cement, and reef sand. I like it because I can always have the perfect piece. No searching for the ones to fit just right. Then you get a bunch of nice looking live rock to seed it and in a few months it’s ALL live rock and hard to tell apart. Well that’s assuming you made natural looking rock that is.
For substrate I think most agree that Crushed coral is on its way out and being used less and less. Personally I don’t like the look and also don’t thinks its as beneficial as sand is.
Substrate is an ongoing debate with things now even taking a turn to no substrate at all.
As for the current "Best " I think live sand is the best route
