What kind of tang? What are you feeding it?

I betcha the tang would fatten up quickly if it could graze on live macro algae (chaetomorphia, caulerpas) all the time.

IMHO, blood worms are garbage.
How much and how often do you feed?
Are you using regular Mysis shrimp or PE Mysis?
What does your live rock look like? Clean?
How old is the tank?
How long have you had the tang?
Can you post a picture of the tang and tank?

I suggest feeding it some healthy food like Emerald Diet Frozen (which is actually made for tangs and other algae loving fish) and Rod's Food. you can throw in some NLS pellets too.
