If you feel that you are getting close to the end then what is the harm in increasing the Pred? Immunosuppressive dose in dog is a whopping 1mg/# BID.
Sorry about your loss of faith in your current vet.
While my hope is that the end is not near, when it comes, I hope for you and him that he passes quietly at home.

Man, I just erased a whole load of stuff on accident.... dang.
So, if a 19lb dog was on 10mg Qday you'd consider that conservative?
Just curious what you know about Tramadol besides it being super addicting?

I love the vets that are willing to come to the house when that time comes. If they will come for that, most of them will come for "hospice" type care as well, you may want to look into that as well. My mom did that with her cat before she finally had to put him down. I think it made him much more comfortable and bought him more time.

The 10mg daily would be a high dose in almost any circumstance but this. I will cya by saying I can't prescribe for him but if I had a 19# Beagle that seemed to be going downhill I would increase the Pred dose to 10mg BID. This will cause a spike in drinking/urination.
Tramadol: I have not found it to be addicting in dogs. I would say that some dogs get much pain relief from it while others, like my dog, just get diarrhea without much pain relief. It is certainly worth a try.
I do not like to change multiple variables at one time with my patients. If I do and get benefit or side effect I am not sure what change in drugs/dose was responsible. If you are picking up antibiotics I would first start those. If no improvement in 1-2 days, then up the Pred dose, if still no better then consider the Tramadol.

I'm so sorry for your loss. I know exactly what that feels like, terrible thing to go through, but at least he's at peace. My sympathies to you and your family today
