You need to do some testing, take your bulbs out and try them 1 by 1 , I would say there should be some kinda of instant warranty from the lfs, It is completely possible you have a bad unit! Test it out , form your own conclusions, talk to the LFS! Good Luck!

Just my 2 cents, with 2 tangs and that wrasse you have a heck of a bio load, How often do you do water changes? have you added and LR after the cycle?( wondeing if you caused a mini cycle) Do you test to detirmine when to do your water change? My point being, you may be letting the bioload go...
