Really sounds like black cyano, which is something I had invade my tank also. All I did was less frequent feedings and lighting time and it went away eventually.

He's fine. Very few common worms will actually prey on any desirable fish/inverts.

Could be a miniature brittle star. They're generally white, but I have seen exceptions. Try and put some food near it's hole and see if it comes out.

Perhaps an oyster? Those usually have mottled shells.

If it moves itself to a different area, just leave it. That is most likely the area it has the best chance of surviving in.
